Special Education Records Requests

All requests for special education records (evaluations and/or IEPs) should be emailed to spedrecords.request@rusd.org or faxed to (262) 631-7182.
 

The following information must be included in your communication in order to ensure a timely response:

  • Full name of student
  • Date of Birth
  • Student’s most recently attended RUSD school, if known
  • Contact information for person submitting request, including the relationship to the student.
  • For requests originating from another district or non-RUSD school in which the student is enrolled, attach a completed copy of an official Records Request form.
  • For all requests not coming from the parent, or another district or school, a completed and signed Release of Information must be attached.
